OCR description
The covered entity (CE), Milestone Electric, reported that employees were the victims of an email phishing attack that compromised the protected health information (PHI) of 4,973 individuals. The PHI involved included names, addresses, dates of birth, and Social Security numbers. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE implemented additional technical safeguards, updated its policies and procedures, and retrained its workforce members on email security.
