OCR description
The covered entity (CE), Cedar County Iowa, reported that employees were the victims of an email phishing attack that affected the electronic protected health information (ePHI) of 1,138 individuals. The ePHI involved included names, Social Security numbers, addresses, drivers’ license numbers, dates of birth, diagnoses/conditions, medications prescribed, financial data, and other treatment information. The CE notified HHS, affected individuals, and provided complimentary credit monitoring services. In response to the breach, the CE implemented additional administrative and technical safeguards. OCR provided technical assistance to the CE.
