The covered entity (CE), the Center for Autism and Related Disorders, reported that multiple employees were the victims of an email phishing attack that affected the electronic protected health information (ePHI) of 1,440 individuals. The ePHI involved included names, addresses, phone numbers, dates of birth, diagnoses, health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE strengthened its administrative, technical, and security safeguards, and retrained workforce members on the proper methods of recognizing and responding to fraudulent email communications.
