Ascend Clinical, LLC

The covered entity (CE), Ascend Clinical, LLC, reported that numerous employees were the victims of an email phishing attack that affected the electronic protected health information (ePHI) of 77,443 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, lab results, and medications prescribed. The CE notified HHS, affected individuals, the media, and provided free credit monitoring, identity theft, and fraud protection services. In response to the breach, the CE implemented additional administrative, technical, and security safeguards and retrained its staff.
