OCR description
Moffitt Cancer Center & Research Institute, the covered entity (CE), reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 95,695 individuals. The ePHI involved included names, dates of birth, addresses, Social Security numbers, financial information, and treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. The CE also set up a complimentary phone number for questions or concerns.
