The covered entity (CE), The Bridge, Inc., reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 16,431 individuals. The ePHI involved included names, addresses, dates of birth, drivers’ license numbers, Social Security numbers, claims information, financial information, diagnoses, and medications prescribed. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring services. In response to the breach, the CE strengthened its administrative and technical safeguards and retrained its staff.
