OCR description
The covered entity (CE), Allergy & Asthma Center, reported that a workforce member inadvertently e-mailed a document that contained the electronic protected health information (ePHI) of 711 individuals. The ePHI involved included names, dates of birth, and health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE sanctioned the workforce members and retrained all employees.
