OCR description
Cedar Falls Community School District, the covered entity (CE), reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 1,445 individuals. The ePHI involved included names, health insurance information, dates of birth, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media.
