OCR description
The covered entity (CE), Urbandale Community School District, reported that its business associate (BA) was the victim of a cyber-attack involving the electronic protected health information (ePHI) of 647 individuals. The ePHI involved included names, health insurance information, dates of birth, diagnoses, and additional treatment information. The CE notified HHS, affected individuals, and the media. OCR provided the CE with technical assistance regarding its policies and procedures.
