OCR description
Bondurant-Farrar Community School District, the covered entity (CE), reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 603 individuals. The ePHI involved included names, health insurance information, claims information, dates of birth, diagnoses, and treatment information. The CE notified HHS, affected individuals, and the media. OCR provided the CE with technical assistance.
