OCR description
The covered entity (CE), Lake County Health Department and Community Health Center, reported that an employee emailed the protected health information (PHI) of 3,815 individuals to unauthorized entities. The PHI involved included names, addresses, dates of birth, and lab results. The CE notified HHS, the affected individuals, and the media. In its mitigation efforts, the CE sanctioned and retrained the responsible employee and revised its policies and procedures.
