OCR description
The covered entity (CE), Seely Enterprises Company, reported that it experienced a cyber-attack that affected the protected health information (PHI) of approximately 16,196 individuals. The PHI involved included names, addresses, social security numbers, medications, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented new processes and technical safeguards to better protect its PHI.
