Northpoint Recovery Holdings, LLC

The covered entity (CE), Northpoint Recovery Holdings, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 1,866 individuals. The ePHI involved included names, addresses, birthdates, Social Security numbers, drivers’ license numbers, financial information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and offered complimentary credit monitoring services. In its mitigation efforts, the CE strengthened its administrative, technical, and security safeguards and trained its employees on the proper methods of recognizing and responding to fraudulent email communications.
