OCR description
University Hospital, the covered entity (CE), reported that it experienced a cybersecurity incident that compromised the protected health information (PHI) of 39,207 individuals. The PHI involved included names, addresses, dates of birth, Social Security and drivers’ license numbers, passport numbers, health insurance information, and other treatment information. University Hospital notified HHS, affected individuals, and the media. In its mitigation efforts, the CE strengthened its administrative, technical, and security safeguards.
