Hi-Tech Artificial Limbs, Inc.

The covered entity (CE), Hi-Tech Artificial Limbs, Inc., reported that a hard drive containing the electronic protected health information (ePHI) of 529 individuals was missing and may have been accessed by an unauthorized individual. The ePHI involved included names, dates of birth, email addresses, home addresses, Social Security numbers, claims information, lab results, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI. OCR provided the CE with technical assistance regarding the HIPAA Breach Notification Rule and obtained assurances that the CE implemented the corrective actions noted.
