OCR description
The covered entity (CE), Golden Gate Regional Center, reported that it was the victim of a ransomware attack that affected the electronic protected health information (ePHI) of 11,315 individuals. The ePHI involved included names, identification numbers, and other treatment information. The CE notified HHS, affected individuals, the media, and provided complimentary credit monitoring and identity theft protection services. Following OCR’s investigation, the CE revised its policies and procedures and provided security awareness training to all employees.
