OCR description
Bardstown Primary Care, PLLC, dba Physicians to Children & Adolescents, the covered entity (CE), reported that its business associate (BA) mailed the protected health information (PHI) of 859 individuals to the wrong recipients. The PHI involved included names, health insurance information, and claims information. The CE notified HHS, affected individuals, and the media. Following the breach, the CE terminated its relationship with the BA.
