OCR description
Tufts Health Plan, the covered entity (CE), reported that an employee of its business associate (BA) was the victim of an email phishing attack that affected the protected health information (PHI) of 60,545 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, and treatment and health insurance information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. OCR provided the CE with technical assistance on its breach reporting requirements.
