OCR description

The covered entity (CE), Brazos Valley Plastic Surgery, reported that an employee inadvertently emailed the protected health information (PHI) of 2,255 individuals to the wrong recipients. The PHI involved included names and addresses. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE retrained its workforce on email security.
