Liv-On Family Care Center, PA

The covered entity (CE), Liv-On Family Care Center, reported that multiple computers and electronic devices were stolen from its facility. This breach affected the protected health information (PHI) of 1,581 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, drivers’ license numbers, claims information, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and physical safeguards to better protect its PHI.
