OCR description
The covered entity (CE), SSM Health Insurance Company now Medica Central Insurance Company, reported that an employee mailed the protected health information (PHI) of 4,492 individuals to the wrong recipients. The PHI involved included names and health insurance information. The CE notified HHS, affected individuals and the media. In response to the breach the CE centralized its outgoing mail process and implemented policies, procedures, and job aids for the relevant workforce.
