The covered entity (CE), Kristina T. Nguyen, DDS, reported that it was the subject of a cyberattack that affected the protected health information (PHI) of 8,182 individuals. The PHI involved included names, addresses, Social Security numbers, dates of birth, medications, health insurance information, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. In addition, the CE retrained its workforce members to better protect its PHI.
