OCR description
Aetna ACE, the covered entity (CE), reported that multiple employees of its business associate (BA) experienced a phishing attack that affected the electronic protected health information (ePHI) of 484,157 individuals. The ePHI involved included names, addresses, Social Security numbers, dates of birth, diagnoses, and financial and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE revised its policies and procedures relating to BAs.
