Agency for Community Treatment Services, Inc.

The covered entity (CE), Agency for Community Treatment Services, reported that it experienced a cyber-attack that compromised the protected health information (PHI) of 73,825 individuals. The PHI involved included names, dates of birth, Social Security numbers, financial and health insurance information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. Further, the CE provided additional training to its workforce regarding best practices for HIPAA privacy and security and the use of the CE’s network systems.
