OCR description
The covered entity (CE), Riverside Community Care, Inc., reported that it suffered a ransomware attack that affected the electronic protected health information (ePHI) of 6,978 individuals. The ePHI involved included names, Social Security numbers, addresses, dates of birth, diagnoses/conditions, medication information, and lab results. In response to the breach, the CE added additional security safeguards to better protect its ePHI. The CE notified HHS, affected individuals, the media, and provided substitute notice.
