OCR description
The covered entity (CE), Magnolia Health Plan, reported that an employee of its business associate (BA) inadvertently mailed documents containing the protected health information (PHI) of 759 individuals to the wrong recipients. The PHI involved included names and health insurance information. The CE notified HHS, affected individuals, and the media. Following the breach, the BA implemented additional administrative safeguards and retrained its staff to better protect PHI. OCR obtained assurances that the CE implemented the corrective actions noted.
