Sunshine State Health Plan

The covered entity (CE), Sunshine State Health Plan, reported that an employee of its business associate discovered a batch processing error in which the protected health information (PHI) of 3,429 individuals was mailed to the wrong recipients. The PHI involved included names, addresses, dates of birth, and health insurance information. The CE notified HHS, affected individuals, the media, and provided a complimentary phone number for questions or concerns. In response to the breach, the BA updated its batch processing software to improve quality controls and strengthened its administrative safeguards to better protect PHI. All workforce members were retrained.
