OCR description
The covered entity (CE), Ambetter for Arizona Complete Health, reported that its business associate (BA) inadvertently mailed documents containing the protected health information (PHI) of 518 individuals to the wrong recipients. The PHI involved included names and health insurance information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the BA strengthened its administrative safeguards to better protect its sensitive data. OCR obtained assurances that the BA implemented the corrective actions noted.
