Prestera Center for Mental Health Services, Inc.

The covered entity (CE), Prestera Center for Mental Health Services, reported that an employee was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 3,708 individuals. The ePHI involved included names, dates of birth, Social Security numbers, health insurance information, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. As a result of the incident, the CE implemented additional technical safeguards and trained its staff on phishing and social engineering.
