OCR description
The covered entity (CE), REEF Global, Inc., reported that it suffered a ransomware attack that affected the electronic protected health information (ePHI) of 3,951 individuals. The ePHI involved included names, dates of birth, addresses, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, and offered complimentary credit monitoring services. In response to the breach, the CE strengthened its administrative and technical safeguards to better protect its ePHI.
