OCR description
The covered entity (CE), KR Perio, Katherine Rodriguez, DMD, reported that it was the subject of a ransomware attack that affected the protected health information (PHI) of 885 individuals. The PHI involved included names, dates of birth, Social Security numbers, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative and technical safeguards to better protect its PHI. OCR provided the CE with technical assistance.
