OCR description
The covered entity (CE), Greater New Bedford Community Health Center, reported that an employee of its business associate was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 696 individuals. The ePHI involved included names, dates of birth, phone numbers, clinical information, and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
