OCR description
Wisconsin Department of Health Services, the covered entity (CE), reported that its business associate (BA) experienced a social engineering attack that compromised the protected health information (PHI) of 1,281 individuals. The PHI involved included names and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. In its mitigation efforts, the CE provided complimentary credit monitoring services and the CE and BA implemented additional administrative and technical safeguards to better protect sensitive data. Staff were retrained on the requirement to protect and secure PHI.
