OCR description
The covered entity (CE), Montgomery Hospice, reported that multiple employees were the victims of an email phishing attack affecting the protected health information (PHI) of approximately 6,741 individuals. The PHI involved included names, medical record numbers, dates of birth, Social Security numbers, and health insurance and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice on its website. In response to the breach, the CE retrained workforce members, offered free credit monitoring to affected individuals, and implemented new technical safeguards.
