OCR description
Merrill Iron & Steel Health Benefit Plan, the covered entity (CE), reported that it experienced a ransomware attack that affected the protected health information (PHI) of 899 individuals. The PHI involved included names, addresses, dates of birth, social security numbers, and claims information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE provided complimentary credit monitoring to affected individuals and implemented additional technical safeguards.
