OCR description
The covered entity (CE), Inova Health System, reported that an employee inadvertently emailed the protected health information (PHI) of 1,680 individuals to the wrong recipient. The PHI involved included names, dates of birth, diagnoses, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and established a call center for questions or concerns. In response to the breach, the CE sanctioned the employee responsible and retrained its staff on the need to safeguard PHI.
