Florida Hospital Physician Group Inc.

The covered entity (CE), Florida Hospital Physician Group, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 13,759 individuals. The PHI involved included names, mailing addresses, dates of birth, drivers’ license numbers, Social Security numbers, financial information, diagnoses, email addresses, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute breach notice on its website. The CE also established a complimentary phone number for questions or concerns and offered credit monitoring services. In response to the breach, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI.
