OCR description
The business associate (BA), Prime Therapeutics, LLC, reported that an employee inadvertently mailed documents containing the protected health information (PHI) of 1,863 individuals to the wrong recipients. The PHI involved included names, addresses, and medication information. The BA notified HHS, affected individuals, and the media. In response to the breach, the BA instituted additional quality assurance checks for mailings and retrained its staff. OCR obtained assurances that the CE implemented the corrective actions noted.
