OCR description
The covered entity (CE), County of Ramsey, reported that its business associate (BA) was the victim of a cyber-attack that affected the electronic protected health information (ePHI) of 8,687 individuals. The ePHI involved included names, addresses, dates of birth, Social Security numbers, claims information, and treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE strengthen its technical safeguards to better protect its ePHI and terminated its relationship with the BA.
