OCR description
The covered entity (CE), A Renewed Mind, reported that an employee was the victim of an email phishing scheme that affected the protected health information (PHI) of 2,428 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses/conditions, medications prescribed, and other treatment information. The CE notified HHS, affected individuals. and the media. In response to the breach, the CE strengthened its administrative and technical safeguards and retrained its staff on email security. OCR provided technical assistance to the CE regarding the HIPAA Rules.
