OCR description
The covered entity (CE), Lake Charles Memorial Health System, reported that an employee of its business associate was the victim of an email phishing scheme that affected the electronic protected health information (ePHI) of 3,802 individuals. The ePHI involved included names, dates of birth, medical record numbers, financial information, and health insurance information. The CE notified HHS, affected individuals, and the media.
