OCR description
The covered entity (CE), Hackley Community Care Center, reported that an employee was the victim of an email phishing attack that compromised the protected health information (PHI) of 2,644 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional technical safeguards to better protect its PHI. OCR provided the CE with technical assistance regarding the HIPAA Rules.
