OCR description
The covered entity (CE), Pennsylvania Adult & Teen Challenge, reported that multiple employees were the victims of an email phishing attack that affected the electronic protected health information (ePHI) of approximately 7,771 individuals. The ePHI involved included names, dates of birth, Social Security numbers, addresses, and clinical and health insurance information. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its ePHI.
