OCR description
The covered entity (CE), Hope Healthcare Service, reported that its business associate (BA) experienced a ransomware attack that compromised the protected health information (PHI) of 1,152 individuals. The PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, and other treatment information. The CE notified HHS. The CE and the BA notified affected individuals and the media. In its mitigation efforts, the CE implemented additional administrative, technical, and security safeguards to better protect its PHI. OCR provided the CE with technical assistance regarding the HIPAA Privacy Rule.
