Bowling Green Inn of Pensacola, LLC DBA Twelve Oaks Recovery

The covered entity (CE), Bowling Green Inn of Pensacola, dba Twelve Oaks Recovery, reported that it was the subject of a ransomware attack that compromised the protected health information (PHI) of 9,023 individuals. The PHI involved included names, addresses and Social Security numbers. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional administrative, technical, and security safeguards, and retrained its workforce on email security.
