OCR description
The covered entity (CE), Miracle-Ear, reported that it was burglarized and the protected health information (PHI) of approximately 2,710 individuals was potentially affected. The PHI involved included names, addresses, birthdates, Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. Free credit monitoring services were also provided to those affected. In its mitigation efforts, the CE strengthened its physical safeguards.
