OCR description
The covered entity (CE), Miracle-Ear, reported that its office was burglarized and the protected health information (PHI) of approximately 1,094 individuals was compromised. The PHI involved included names, addresses, birthdates, Social Security numbers, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. Free credit monitoring services were offered, and a dedicated phone line was established for questions or concerns. In response to the breach, the CE implemented additional physical safeguards.
