OCR description
The covered entity (CE), Clovis Community Medical Center, reported that an employee impermissibly sent an email that contained the electronic protected health information (ePHI) of 994 individuals to an external personal email account. The ePHI involved included names, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media. In response to the breach, the CE implemented additional administrative safeguards and retrained its staff.
