The SurgiCare Center of Utah

The covered entity (CE), the SurgiCare Center of Utah, reported that it experienced a ransomware attack that compromised the protected health information (PHI) of 8,816 individuals. The PHI involved included names, Social Security numbers, dates of birth, addresses, diagnoses, lab results, medications prescribed, and other treatment information. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE provided complimentary credit monitoring and identity theft protection services and strengthened its administrative, technical, and security safeguards to better protect its PHI. OCR provided technical assistance to the CE regarding its HIPAA Security Rule obligations.
