OCR description
The covered entity (CE), Geisinger Health Plan, reported that due to a mailing error, the protected health information of 2,872 individuals was viewable on a cover letter. The CE notified HHS, affected individuals, and the media. In its mitigation efforts, the CE implemented additional administrative safeguards to better protect its PHI.
